门冬氨酸钾镁在心脏瓣膜体外循环术中对CK-MB和cTnT的影响

Effect of potassium magnesium aspartate on CK-MB and cTnT in patients undergoing heart valve replacement under extracorporeal circulation

摘要 目的观察体外循环(ECC)下行心脏瓣膜术中应用门冬氨酸钾镁(PMA)对肌酸激酶同工酶(CK-MB)和心肌肌钙蛋白T(cTnT)的影响。方法 40例择期心脏瓣膜置换患者随机分成PMA组与对照(C)组,每组20例。PMA组心肌保护液维持Mg2+浓度为4mmol/L;C组心肌保护液中不含Mg2+。监测术前、术毕、术后6、24、72hCK-MB和cTnT值。结果两组术后CK-MB和cTnT均比术前明显升高(P<0.05)。PMA组术毕、术后6和24h的CK-MB明显低于C组[(42.1±8.3)IU/Lvs.(77.4±12.5)IU/L、(30.6±5.8)IU/Lvs.(53.2±9.5)IU/L、(16.4±6.3)IU/Lvs.(37.4±10.8)IU/L](P<0.05);PMA组cTnT也明显低于C组[(0.31±0.08)μg/Lvs.(0.93±0.25)μg/L、(0.55±0.10)μg/Lvs.(1.14±0.21)μg/L、(0.68±0.12)μg/Lvs.(1.37±0.36)μg/L](P<0.05)。结论门冬氨酸钾镁在心脏瓣膜病体外循环手术中有心肌保护作用。 Objective To investigate the effects of potassium magnesium aspartate(PMA)on creatine kinase-MB(CK-MB)and calcitonin-T(cTnT)during heart valve replacement under extracorporeal circulation(ECC).Methods Forty patients undergoing heart valve replacement were randomly divided into two groups with 20 cases each.The magnesium 4 mmol/L in PMA cardioplegia solution was added in group PMA,which was not in group C as the control.Plasma CK-MB and cTnT were detected before operation,at the end of surgery,6,24 and 72h after operation.ResultsCompared to before,plasma CK-MB and cTnT were significantly increased after operation(P0.05).Plasma concentrations of CK-MB at the end of surgery,6 and 24h after operation were significantly lower in group PMA than those in group C[(42.1±8.3)IU/L vs.(77.4±12.5)IU/L,(30.6±5.8)IU/L vs.(53.2±9.5)IU/L and(16.4±6.3)IU/L vs.(37.4±10.8)IU/L](P0.05).So did cTnT [(0.31±0.08)μg/L vs.(0.93±0.25)μg/L,(0.55±0.10)μg/L vs.(1.14±0.21)μg/L and(0.68±0.12)μg/L vs.(1.37±0.36)μg/L](P0.05).Conclusion The magnesium 4mmol/L added into cardioplegia solution has myocardial protective effect in the patients undergoing heart valve replacement under ECC.
