几种不同降压药物用于脊柱外科手术控制性降压麻醉的临床研究 被引量:3

摘要 目的对比观察硝酸甘油、瑞芬太尼、硝酸甘油复合瑞芬太尼用于脊柱外科手术控制性降压麻醉的安全性和有效性。方法选择全麻下行脊柱手术患者45例,分别采用硝酸甘油(I组)、瑞芬太尼(II组)、瑞芬太尼复合硝酸甘油(Ⅲ组)控制性降压麻醉,维持患者MAP在60~70mmHg,观察三组患者控制性降压前即刻、降压后5、10、30、60min的MAP和HR及降压停止后MAP恢复到降压前水平的时间。结果三组患者控制性降压后MAP均显著下降(P〈0.01),和降压前即刻相比较,I组降压后HR较降压前有明显增快(P〈0.05);而II组患者的HR有明显下降(P〈0.05),且随着药物剂量增加有下降更明显的趋势。Ⅲ组降压前后HR变化不大。降压停止后血压恢复至降压前水平时间Ⅱ组(12±3)min和Ⅲ组(10±2)min,明显快于I组(304-2)min。结论瑞芬太尼复合硝酸甘油用于脊柱外科手术控制性降压麻醉,可取长补短,安全,效果可靠。
