摘要
目的观察半乳甘露聚糖(GM)在大鼠侵袭性肺曲霉菌感染(IPA)模型不同体液标本不同时间点检测的意义。方法建立大鼠IPA模型,收集不同时间的血浆、支气管肺泡灌洗液(BALF)和尿液标本,检测GM含量,同时进行肺组织病理检查和真菌学培养。结果实验组感染第1、3、5天血浆GM实验平均敏感性分别为58.30%、66.67%、83.33%,BALF分别为66.67%、91.67%、100%,与对照组比较均有统计学差异。所有72只实验大鼠的血浆与BALF标本GM敏感性分别为69.44%、86.11%,特异性分别为100%、100%,阳性预测值分别为100%、100%,阴性预测值分别为68.57%、82.76%。尿液GM检测共5例阳性,敏感性13.89%,特异性100%。血浆及BALF检测GM值随感染天数增加呈升高趋势。结论BALF标本GM检测与血浆标本GM检测具有同样意义,均有助于IPA的早期诊断。
Objective To assess the value of galactaomannan(GM) detection in three kinds of fluid samples from rat model of invasive pulmonary aspergillosis(IPA).Methods Collected the samples of bronchoalveolar lavage fluid(BALF),plasma and urine for GM detection after the IPA model was established.At the same time,the lung tissue was collected for pathological examination and mycology culture.Results After early infection for 1,3 and 5 days,the average sensitivity of GM test of plasma in experimental group were 58.30%,66.67% and 83.33%,while,the GM test of BALF were 66.67%,91.67% and 100%,respectively.That had significant differences compared with control group.All of the 72 rats' GM value in plasma and BALF samples had the sensitivity for 69.44%,86.11%,and both the specificity for 100%.Meanwhile,the positive predictive value were both 100%,and the negative predictive value were 68.57% and 82.76%,respectively.There were 5 positive GM value in all urine specimens detection with the sensitivity was 13.89%,and the specificity was 100%.All the GM value trended to rise with the increase of infection days.Conclusion GM detection in BALF and plasma may contribute to the early diagnosis of IPA.
