摘要
目的探讨应用臀大肌肌皮瓣修复骶尾部软组织缺损的I临床疗效。方法应用单侧臀大肌上部、下部肌皮瓣，单侧全臀大肌肌皮瓣，双侧臀大肌旋转推进皮瓣转移修复骶尾部软组织缺损16例；其中9例褥疮及1例长期窦道患者，先行清创和负压封闭引流手术控制感染，再行肌皮瓣转移手术。结果所有肌皮瓣均全部完全成活，伤口I期愈合。随访6个月～5年，仅1例出现轻度伸髋障碍，其余患者外形及功能均满意，无复发。结论应用臀大肌皮瓣转位治疗骶尾部软组织缺损是较理想的手术方法。
Objective To evaluate the clinical effects of gluteus maximus myocutaneous flap （GMMF） for reconstruction of sacral sacrococcygeal soft tissue defects. Methods Unilateral inferior GMMF, Unilateral superior GMMF, unilateral total GMMF and bilateral V - Y advancement GMMF were applied to repair the sacrococcygeal soft tissue defects for 16 patients. For 9 cases of pressure ulcers and 1 case of long - term sinus, debridement and vacuum sealing drainage were carried out firstly and myocutaneous flaps were transferred to cover the wounds after infections were controlled. Results All myocutaneous flaps were survived completely and phase I healing was acquired. Followed up for six months to 5 years, there was only 1 case with slight hip extension disorder and the others were satisfied with the appearance and function without recurrence. Conclusion GMMF is an ideal surgical method for reconstruction of sacrococcygeal soft tissue defects.
