摘要
目的 探讨保留十二指肠的胰头切除术（DPPHR）在治疗胰头肿瘤上的安全性.方法 回顾性分析我院2008年5月至2009年8月施行该术式7例患者临床资料.结果 无手术死亡.术后发生胰瘘2例,经非手术治疗后痊愈.无胰腺假性囊肿及腹腔脓肿形成,总并发症发生率28.6%.术后7例患者均随访,无术后糖尿病、胆总管狭窄、慢性消化不良发生,胰岛素瘤患者血糖恢复至正常.结论 对于胰头良性及低度恶性肿瘤,DPPHR是一种安全有效的手术方式.
Objective To investigate the safety of duodenum - preserving pancreatic head resection（DPPHR） for pancreatic head tumor, and summarize the experience. Methods Clinical data of 7 patients with pancreatic head resection who were treated by DPPHR from May 2008 to August 2009 were retrospectively analyzed. The details of the surgical procedure and complications were explored and the quality of life after the surgery was assessed. Results There was no operative death. Postoperative pancreatic fistula occurred in 2 patients and was cured by conservative treatment. No pancreatic pseudocyst and abdominal abscess were found. The incidence of postoperative complications was 28.6%. All of patients were followed up. No patients developed diabetes mellitus, stricture of bile duct and chronic dyspepsia. The blood sugar of the patients with insulinoma recovered to the normal level. Conclusion DPRHP is a safe and effective procedure for the treatment of the benign or the low potential malignany pancreatic head tumor.
