摘要
目的 探讨明视负波反应(PhNR)与视网膜厚度分析(RTA)是否能够诊断早期原发性开角型青光眼(POAG)及其各自特性.方法 采用回顾性病例系列对照研究,通过对早期POAG组30例(52只眼).正常对照组40例(80只眼)进行PhNR与RTA检查,比较两种检查的各项指标在早期POAG检测中敏感性、特异性、ROC曲线下面积.结果 对照组和早期POAG组的PhNR振幅比较差异有统计学意义(P〈0.01);对照组和早期POAG组的RTA各项检查指标比较差异有统计学意义(P〈0.01);PhNR的敏感性和特异性分别为87.5%和76.9%;视盘三维形态参数的敏感性分别为75.0%、71.3%、76.3%、82.5%,特异性分别为62.8%、58.7%、65.3%、69.4%;盘周和黄斑区视网膜厚度的敏感性分别为80.0%、83.8%,特异性分别为68.6%、66.1%;PhNR的ROC曲线下面积为0.880,RTA检查各项参数的ROC曲线下面积分别为0.756、0.715、0.767、0.813、0.808、0.831.结论 PhNR与RTA检查均可以诊断早期POAG;PhNR比RTA更敏感,更特异,说明PhNR可以早期发现POAG损害,联合检查将更有助于POAG的早期诊断.
Objective To evaluate photopic negative response (PhNR) and retinal thickness analysis, the possibility of early diagnosis of, and their diagnostic value and respective characteristics in primary open angle glaucoma (POAG). Methods Through the retrospective case series, 30 patients (52 eyes) of the early POAG and 40 cases (80 eyes) of normal eyes were examined with PhNR and RTA examination. The sensitivity, the specificity, and the area under the ROC curve were compared, and analyzed the dependability on PhNR and RTA results. Results There were significant differences between the normal and the early POAG group in PhNR amplitude (F=162.82, P〈0.01). In the normal and the early POAG group, there were significant differences among each RTA parameter value, (P〈0.01). The sensitivity and specificity of PhNR were 87.5% and 76.9%. The sensitivity and 62.8% 、58.7%、65.5%、69.4% .The sensitivity and specificity of optic disk parameters were 75.0%, 71.3%, 76.3% , 82.5% and specificity of mean retinal nerve fiber layer thickness and foveola average thickness were 80.0%, 83.8% and 68.6%, 66.1%. The area under the receiver operator characteristic curve for PhNR and each parameter for RTA were 0.880, 0.756, 0.715, 0.767, 0.813, 0.808, 0.831 respectively .Conclusions PhNR and RTA can diagnose the early POAG. However PhNR has more sensitivity and specificity than RTA. PhNR could detect the early injury of POAG. The combined examination of PhNR and RTA can conduce to the early diagnosis on POAG.
