摘要
目的:为单纯性胸腰段骨折患者寻找一种简单且并发症少的保守治疗方法。方法:90例单纯性胸腰段压缩骨折病人随机分为对照组与治疗组各45例,对照组采用垫枕+手法复位+垫枕维持与腰背肌功能锻炼治疗,治疗组采用床头简易体位+手法复位、体位加垫枕维持与腰背肌功能锻炼治疗,治疗前后分别摄片对比,对伤椎高度和Cobb角的变化进行比较,并观察卧床治疗期间的并发症与下床活动情况。结果:治疗后,两组治疗后的椎体高度比与后凸Cobb角与治疗前比较,差异均有统计学意义(P<0.01),治疗组治疗后的椎体高度比与后凸Cobb角与对照组相比,差异均有统计学意义(P<0.05)。治疗组卧床期间的并发症(呼吸道感染与头晕)比对照组少(P<0.05),下床活动情况(头晕与行走)更佳(P<0.05)。随访6~34月,两组疗效经过Ridit分析,差异均有统计学意义(P<0.05)。结论:床头简易体位复位合手法复位在胸腰椎段骨折中有较肯定的治疗作用,并发症少,下床恢复活动快,值得推广。
Objective:To explore a simple conservative treatment for simple thoracic-lumbar vertebral compression fracture with few complications.Methods:Ninety patients with simple thoracic-lumbar vertebral compression fracture were evenly randomized into control group,who were received lumbar pad,manipulative reduction plus functional exercise,and treatment group,who were treated with proper bedside position,manipulative reduction,lumbar pad and functional exercise of the lumbar muscle.After treatment,the curative effect,the complications,the Centrum height,the Cobb angle and the activities out of bed were observed.Results:Compared with before treatment,the Centrum height rate in both groups increased significantly(P0.01),and the Cobb angle reduced significantly(P0.01).There were significant differences in Centrum height rate and the Cobb angle between treatment group control group(P0.05).There were fewer complications such as respiratory infections and dizziness lying in bed in treatment group than those in control group(P0.05),and there were also better activities out of bed,such as dizziness and walking,in treatment group than that in control group(P0.05).Based on a follow-up of 6-34 months,the curative effect in two groups showed significant difference(P0.05).Conclusion:Manipulative reduction combined with bedside position is effective for simple thoracic-lumbar vertebra compression fracture,with few complications and early activities out of bed,which deserves more application.
