摘要
目的准确筛查出染色体异常的胎儿是产前诊断的重要环节,目前一些反映胎儿染色体异常的超声指征逐渐受到重视。文中利用超声检测中孕期胎儿鼻骨发育,探讨其对发现胎儿染色体畸形的临床价值。方法选取2007年10月至2009年5月行超声检查的单胎孕妇2519例,从胎头矢状面测量鼻骨长度,及其他胎儿生物参数,对超声检查鼻骨异常者行染色体核型分析。结果胎儿鼻骨长度和其他生物参数均成功测及,鼻骨长度与孕龄、双顶径呈线性相关(P<0.05)。15例胎儿为鼻骨异常并伴有其他畸形,经行染色体核型分析其中14例为三体儿。结论中孕期超声检查胎儿鼻骨对发现染色体异常有一定的实用价值。
Objective It is important to detect fetal chromosomal abnormality in prenatal examination,some ultrasound indications were found to reflect chromosomal abnormality.This study is to investigate the clinic value of ultrasonographic fetal nasal bone(NB) examination for chromosomal defects risk assessment in the second trimester.Methods A total of 2519 women with a singleton pregnancy who registered from Oct 2007 to May 2009 was reviewed.Ultrasound measurements of nasal bone lenth(NBL) were performed at strict sagittal plan of the fetal head.Other fetal biometry profiles were also conducted and chromosome kasyotype analysis was taken in one with abnormal nasal bone.Results NBL and fetal biometry profiles were measured successfully in 100% of the fetuses.There were linear correlation between NBL and gestation,biparietal diameter(BPD),respectively(P0.05).Fifteen fetals were abnormal nasal bone with other malformations,among which 14 were trisomy fetuses detected by chromosome kasyotype analysis.Conclusion Evaluating fetal nasal bone with ultrasound in the second trimester scan is valuable for determining chromosomal abnormality.
