摘要
目的:探讨眼外肌手术量的设计。方法:收集本科室2003年3月～2009年3月进行眼外肌的手术病员,分析其手术并总结手术量的设计。结果:内直肌截除或后徙1 mm约矫正1.5°斜视角,外直肌后徙或截除1 mm约矫正1°斜视角,截除并后徙1 mm约矫正3°斜视角,下直肌后徙在第一眼位为每毫米1.5°,向下注视眼位为每毫米2.5°,垂直肌后徙或缩短1 mm最多矫正1.5°,一般后徙量不超过6 mm,直肌截除量,内直肌7～8 mm,外直肌10 mm,上下直肌各6 mm。结论:从临床经验中总结眼外肌手术量的设计,为今后的眼外肌手术提供参考,但眼外肌手术不能一概公式化,具体还应结合患者的实际情况和要求来设计。
Objective： To study how to arrange the positions for the extrinsic eyeball muscles. Method： 135 cases with extrinsic eyeball muscles treated by surgery from 2003, Mar to 2009, Mar were reviewed and summarized, and their pathogenic factors and operation program were analyzed. Results： Remove the musculus rectus medialis backwards approx lmm or amputate the muscular tissue approx 1mm to correct the visual acuity with 1.5-degree angle of strabismus, remove the musculus rectus medialis backwards approx lmm or amputate the muscular tissue approx 1mm to correct the visual acuity withl-degree angle of strabismus, and remove the musculus rectus medialis backwards approx lmm or amputate the muscular tissue approx lmm to correct the visual acuity with 3- degree angle of strabismus. The inferior rectus muscle shall be kept at 1.5 degree/ram after removing backwards to the first ocular position, and be kept at 2. 5 degree/ram at the ocular position with downward visual fixation. The vertical rectus removing backwards lmm or decurtating lmm shall correct 1.5 degree angle of strabismus at most. Conclusion： The ocular surgery for the extrinsic eyebaU muscles can not be carried out with the forrnulized therapeutic program, which should he prepared and confirmed according to the inpatient＇s conditions and the state of the illness.
