摘要
目的探讨双切口双钢板法治疗胫骨平台复杂性骨折的临床疗效。方法2004年2月～2008年11月我院收治19例胫骨平台复杂性骨折的患者,根据Schatzker分型:V型9例,VI型10例。采用双切口双钢板的方法进行手术治疗,所有患者术后第1天起即进行膝关节功能锻炼。结果所有患者术后获得6～32个月(平均17.2个月)随访,骨折均获得骨性愈合,骨折愈合时间平均4.5个月。膝关节功能采用Merchant评分标准评定:优11例,良5例,差3例,优良率84.2%。结论采用双切口双钢板法治疗胫骨平台复杂性骨折,具有手术创伤小,切口及软组织并发症少,骨折固定长期的稳定性良好,可以早期行CPM机锻炼,关节功能恢复满意等优点,是治疗胫骨平台复杂性骨折安全、有效的方法。
Objective To explore the clinical effects of treating complex tibial plateau fractures with two incisions and double plates fixation.Methods Between February 2004 and November 2008,19 patients with complex tibial plateau fractures were surgically treated with two incisions and double plates fixation in our hospital as Schatzker V（9 cases）and Schatzker VI（10 cases）.Exercises to recover the function of knee joint went on gradually from the first day after operation for all cases.Results All patients were followed up for 6~32 months with an average of 17.2 months.All patients achieved bone union.The duration of union averaged 4.5 months.Knee joint function was rated based on Mechant evaluation system as excellent in 11 cases,good in 5 cases and fair in 3 cases.The excellent rate was 84.2%.Conclusion The treatment of complex tibial plateau fractures with two incisions and double plates fixation has advantages of causing little operative trauma,a low rate of incision and soft tissue complication,a long stablized good fracture fixation,an early feasibility of exercise with CPM to recover knee joint function.It is a safe and effective method to treat complex tibial plateau fractures.
