摘要
目的探讨肾囊肿后腹膜腔镜手术的并发症及其处理。方法采用后腹膜腔镜手术治疗肾囊肿116例,其中囊肿位于右侧49例,左侧67例,肾上极45例,中部23例,肾下极48例。囊肿大小3.3 cm×3.5 cm×4.0 cm-10.4 cm×8.2 cm×9.8 cm,均一次性完成手术。结果 116例中13例有不同程度的残腔存留和复发;2例肾盏憩室术前诊断明确,术中术后各发现1例,术中发现1例行开放手术修补,另1例术后出现尿漏,经双"J"管引流2个月后好转;盂旁囊肿4例,1例出现短暂漏尿;多发囊肿2例,1例多房囊肿术后引流血液量较多,延迟拔管。结论后腹膜腔镜手术治疗肾囊肿安全有效,但应提高认识,积极治疗和预防并发症的发生。
Objective To investigate complications and handlings of laparoscopic operation in treatment of renal cysts.Methods The clinical data of 116 cases of cysts of kidney treated by retro-peritioneal laparoscopic unroofing were retrospectively analyzed.There were 49 cases of right cysts,67 cases of left cysts,45 cases of upper cysts,23 cases of middle cysts and 48 cases of lower cysts.The cysts,not equally in the same size,approximately ranging from 3.3×3.5×4.0 cm to 10.4×8.2×9.8cm,were all treated in one operation.Results Among 116cases,there were 13 cases with incomplete cavity and recurrence;two cases with explicit dignosis of calyceal diverticulum before the operation;one case with diagnostic calyceal diverticulum during the operation was converted to open operation for repair;another case with diagnostic calyceal diverticulum after the operation had leakage of urine and recovered after two-month drainage with the Double J Tube;there are 4 cases of parapelvic cysts,one case had temporary leakage of urine,2 cases had multilocular cyst,one case with compound cyst had drainage of much blood after operation and was delayed the tube removal.Conclusion Laparoscopic operation in treatment of renal cysts is effective with the improvement of operating skills,the accumulation of operating experiences,the awareness and prophylaxis of the complications.
