摘要
目的分析乙肝病毒(HBV)不同血清标志物(HBVM)中丙酸氨基转移酶(ALT)异常率与乙肝病毒脱氧核糖核酸(HBVDNA)载量的关系,探讨乙肝病毒感染时肝损伤的主要原因及检测HBVM与HBVDNA载量及ALT的临床意义。方法对345份乙肝患者的血清标本分别用ELISA、荧光定量PCR、速率法检测HBVM、HBVDNA载量及ALT。结果①HBeAg(+)与HBeAg(-)两组HBVDNA阳性率、HBVDNA载量>105的百分率比较均有显著差异(P<0.01)。②ALT异常率在HBeAg(+)组的不同HBVDNA载量级中无差异(P>0.05),在HBeAg(-)组中随HB-VDNA载量级增大而增大(P<0.01)。结论在HBeAg(+)时,肝损伤与HBVDNA载量无关;HBeAg(-)时,大多病毒复制减弱,但有少数HBVDNA载量为高拷贝,其肝损伤程度与HBVDNA载量呈正相关。
Objective To explore the relationship between the rate of HB patients with abnormal ALT and quantity of HBV. Method 34,5 patients with HB were examined for HBV marker, quantity of HBVDNA and ALT by ELSIA, flourescence quantity PCR, and chemical rate method, respectively. Result The rate of HBV-DNA (+) and quantity of HBVDNA more than 10s in HBeAg (+) group were higher than that in HBeAg (-) group. The rate of abnormal ALT in HBeAg (+) group in different quantity of HBVDNA was not significantly different. The rate of abnormal ALT in HBeAg (-) group was increased according to the quantity of HBVDNA. Conclusion The level of hepatocellular damage of HB with HBeAg positive models in serum was not correlation to activity of virus reproduction, but level hepatocellular damage of HB with HBeAg negative models was positive correlateion to the load quantity of HBV.
