辐射保暖台治疗新生儿高胆红素血症的效果观察 被引量:5

摘要 目的探讨辐射保暖台对新生儿高胆红素血症的光疗效应。方法将69例高胆红素血症新生患儿按随机数字表法分为观察组(n=36)和对照组(n=33),观察组患儿采用辐射保暖台进行治疗,对照组采用单面蓝光床进行治疗。比较两组患儿血清总胆红素(total bilirubin,TBIL)、直接胆红素(direct bilirubin,DBIL)和间接胆红素(indirect bilirubin,IBIL)的水平,应用高效液相色谱仪(high performance liquid chromatography,HPLC)测定血中胆红素异构体,同时观察皮疹、体温、大便性状等改变情况,从而评价辐射保暖台的治疗效果。结果两组患儿治疗后TBIL、DBIL和IBIL水平均下降,与治疗前相比差异有统计学意义(P<0.01)。观察组患儿治疗后TBIL、DBIL、IBIL水平低于对照组治疗后水平(P<0.01)。光源照射6 h后,两组患儿血清胆红素构象异构体ZE(14Z,15E-bilirubin IXa)、EZ(14E,15Z-bilirubin IXa)差异均有统计学意义(P<0.05或P<0.01),而两组患儿血清胆红素结构异构体LR(lumirubin)的差异无统计学意义(P>0.05);光照12 h后,两组患儿血清EZ、ZE值的差异均有统计学意义(P<0.05或P<0.01),两组患儿血清LR值之间差异无统计学意义(P>0.05)。观察组患儿未发现皮疹、腹泻及发热,对照组有个别患儿出现发热和腹泻等症状。结论辐射保暖台治疗后新生儿血清胆红素异构化显著,其效果较普通单面蓝光床更好,是值得推广的一种新治疗光源。 Objective To evaluate the phototherapeutic effects of babytherm for neonatal hyperbilirubi- nemia. Methods Sixty-nine jaundiced newborns were randomized into the observational group (n = 36) treated with babytherm and the control group (n = 33) irradiated by single side conventional blue light (SCBL). A comparison was conducted to measure the levels of total bilirubin (TBII.), direct bilirubin (DBIL) and indirect bilirubin(IBIL) in the patients of both groups. The quantity of serum bilirubin isomers were detected during phototherapy with high performance liquid chromatography(HPI.C). Meanwhile the changes of rash,stool consistency and temperature were also observed to evaluate the effects of babytherm. Results After the treatment,the levels of TBIL,DBIL and IBIL were significantly decreased in all the pa- tients(P〈0.01);The levels of TBIL,DBIL and IBIL were significantly lower in the observational group than in the control group(P(0.01). After 6 h of phototherapy,there were significant differences between the serum(14Z, 15E-bilirubin IXa)and EZ(14E, 15Z-bilirubin IXa) value of the observational group and the control group(P〈0.01 or P〈0.05) ;While no significant difference in the serum LR values was observed between the two groups(P〉0.05). After 12 h of phototherapy,there were significant differences in the se rum EZ and ZE value within two groups(P〈0.05) ;while no significant difference in the serum I.R values were observed between the two groups(P〉0.05 or P〈O. 01). No adverse reactions such as rash,diarrhea and fever were observed in the observational group. Only individual outbreaks of fever and diarrhea were found in the control group. Conclusion As a promising novel source of light, babytherm is characterized by significant bilirubin photoisomerization and is more effective during phototherapy than SCBL.
