摘要 目的探讨血氧水平依赖功能磁共振成像(BOLD-fMRI)对皮层运动区肿瘤切除术的临床应用价值。方法BOLD-fMRI方法采用复杂对指运动,施行皮层运动区脑肿瘤手术64例,运用术中影像数据对脑肿瘤与脑功能区之间的距离及其与术后并发症的关系作回顾性分析。结果术前BOLD-fMRI均能成功定位皮层运动区,脑肿瘤与皮层运动区之间的距离小于10mm的患者术后并发症明显增加(P<0.05)。结论BOLD-fMRI是一种无创有效的皮层运动区定位方法,对术前皮层运动区肿瘤手术计划的制定、术中对于功能区的保护和预测术后并发症的发生有重要临床价值。 Objective To explore the clinical value of blood oxygen level dependent functional magnetic resonance imaging( BOLD-fMRI)to the microsurgery for motor cortexes tumors.Methods BOLD-fMRI of brain motor cortexes was obtained by complexf inger movement.Neurosurgery was performed in 64 patients with tumors in the motor cortexes or near them.The relationship of thed istance(D)from the tumors to the motor cortexes with the postoperative complication was analyzed.Results The motor cortexes werei dentified by preoperative BOLD-fMRI in all the patients.The occurent rate of postoperative complication in the patients with the tumors of D≤10 mm was significant higher than that in the patients with the tumors of D10 mm(P0.05).Conclusions BOLD-fMRI,which is an effective and non-invasive method to localize the motor cortex,is of clinical value to making surgery planning,increasingi ntraoperative motor cortex protection and decreasing the postoperative complication.
