摘要
目的:评估中医外治法对寰枢椎骨关节炎的临床疗效。方法将36例确诊为寰枢外侧关节骨关节炎患者分为2组:对照组18例,单纯口服芬必得胶囊;治疗组18例,除服用芬必得胶囊以外,加用中医外治法。评估两组患者治疗前后的疼痛程度、活动范围及影像学变化,观察比较其疗效。结果:两组患者治疗后VAS评分分别与治疗前比较均有下降,差异有统计学意义(P<0.05);并且治疗组评分下降(6.7±1.8)分,较对照组的(4.4±2.4)分明显,二者间差异有统计学意义(P<0.05)。治疗组的治愈率为77.8%,好转率为16.7%,无效率为5.5%;对照组的治愈率为33.3%,好转率为33.3%,无效率为33.3%,两组间疗效比较差异有统计学意义(P<0.05)。结论:中医外治法是寰枢椎骨关节炎的有效治疗方法,口服芬必得胶囊加中医外治法较单用芬必得胶囊治疗效果好。
Objective:To evaluate the clinical outcomes of TCM external treatment for atlantoaxial osteoarthritis.Methods:36 Atlantoaxial osteoarthritis patients were divided into a treatment group(18 cases) and a control group(18 cases).Patients in the treatment group were given oral western medicine and external treatment of TCM,including external medicine and osteopathtic manipulation;while those in the control group were given only oral western medicine treatment.All the patients were assessed with visual analogous scale(VAS) before and after treatment.Results:The pain of all patients was decreased after treatment.The VAS score in both groups were reduced after treatment and the difference was significant as compared with that before treatment(P〈0.05).The decrease of VAS score was(6.7 ±1.8)and(4.4±2.4)in the treatment group and control group respectively.The decrease of headache days per month was(8.9±1.5)and(5.4±1.7)in the treatment group and control group respectively.There was statistically significant difference between the two groups in terms of the decrease of VAS score and headache days per month(P〈0.05).In treatment group the cure rate,improved rate and inefficacy rate were 77.8%,16.5% and 5.5%,respectively,while those in the control group were 33.3%,33.3% and 33.3%.There was statistically significant difference between the two groups(P〈0.05).Conclusion:The use of external therapy of traditional Chinese medicine for atlantoaxial osteoarthritis had better effect than NSAIDs medicine.
