摘要
对108例半侧面肌痉挛患者作了术前桥小脑角区直接增强CT扫描,将CT表现与手术结果进行对比分析。结果:98例手术证实面神经根部有异常血管压迫,占9074%(98/108),其中73例(7449%)CT扫描可见患侧桥小脑角有扩张增粗的血管影;其它病因有桥小脑角占位病变,其中胆脂瘤4例(37%),听神经瘤2例(185%)。结果表明:半侧面肌痉挛最常见的原因是异常血管袢压迫面神经根部。CT在病因诊断方面很有帮助,它既能发现桥小脑角占位病变又能显示桥小脑角扩张增粗的血管。
One hundred and eight patients with hemifacial spasm underwent direct enhanced CT scanning at the cerebellopontile angle before operation. The results of CT and operation were compared. The results showed: Most common cause of hemifacial spasm is vascular compression of the root of facial nerve. Ninty eight patients (90 74%) were found at operation to have vascular compression of the facial nerve. Seventy three of these 98 cases (74 49%) had an ectatic blood vessel at the cerebellopontile angle found by CT. The other causes are space occupying lesions, which include four cholesteatomas (3 7%) and two acoustic neuromas (1 85%). CT is useful in identifying causes of these diseases. It not only can demonstrate space occupying lesions, such as cholesteatoma and acoustic neuroma, but also can reveal the ectatic blood vessels at the cerebellopontile angle, CT therefore provides image materials very helpful for surgery.
