摘要
目的探讨局部组合皮瓣在创面修复中的设计与应用。方法采用局部组合皮瓣修复创面21例,其中7例采用带蒂皮瓣和推进皮瓣组合,4例采用双推进皮瓣,8例采用旋转皮瓣和推进皮瓣组合,1例采用游离皮瓣加旋转皮瓣,1例采用四叶旋转皮瓣。结果21例均获随访,时间6～24个月,皮瓣均成活;1例骨髓炎者局部渗液,1例皮缘坏死,经换药后延期愈合。结论局部组合皮瓣很好地解决了软组织缺损创面的受区和供区同时修复问题,具有操作简单、成活率高、减少创伤和瘢痕等特点。
Objective To study the design and application of local flaps combination for repairing wounds and soft tissue defects.Methods Local combined flaps were designed and applied to repair wounds in 21 cases,of which 7 cases were pedicle flaps and forward flaps,4 double forward flaps,8 rotating flaps and forward flaps,1 free flaps and rotating flaps,and 1 case a four-leaf rotating flaps.Results 21 cases were followed up for 6~24 months.All flaps survived.1 case of osteomyelitis was complicated by local exudate,and 1 with skin flap necrosis.Both got delayed healing after dress changing.Conclusions Combination of local flaps can solve problem of fixing soft tissue wounds of the affected area or that the donor site at the same time.It has advantages as simple operation,high survival rate,reducing trauma and scar characteristics.
