摘要
目的探讨果糖胺(GSP)及糖化血红蛋白(HbA1C)在妊娠糖尿病(GDM)诊断中的临床应用。方法对正常妊娠组80例及GDM组64例进行空腹血糖(FBG)、口服50g葡萄糖筛查试验、GSP和HbA1C的测定。结果 GDM组FBG、50g糖筛查试验、GSP、HbA1C结果与正常妊娠组比较,差异有统计学意义(P<0.05)。GDM组FBG、口服50g葡萄糖筛查试验、GSP、HbA1C阳性率分别为53.13%、78.13%、82.81%、87.50%。HbAlC其灵敏度、特异度、总的符合率及阳性预测值分别为87.50%、97.50%、93.06%、95.55%,而GSP其灵敏度、特异度、总的符合率及阳性预测值分别为82.81%、92.50%、88.19%、89.83%。在诊断GDM的四项检查中最理想为HbAlC,其次为GSP。结论 HbA1C和GSP在妊娠糖尿病诊断中具有重要意义。
Objective To explore the clinical value of fructosamine and glycosylated hemoglobin in the diagnosis of gestational diabetes mellitus(GDM). Methods The determination of fasting blood glucose(FBG), fructosamine and HbA 1C and the oral administration of 50g glucose screening test were carried out in normal pregnancy group(80 cases)and GDM group(64 cases). Results There were significant differences in the results of FBG, fructosamine, qbAlc and oral administration of 50g glucose screening test between the GDM group and the normal gestation group(P〈0.05 ). The positive rate of the fasting blood sugar,fructosamine,oral administration of 50g glucose screening test and HbA1C were 53.13%, 78.13%, 82.81% and 87.50%. The sensitivity, specificity, total consistent rate and positive predictive value of HbAlc were 87.50% ,97.50% ,93.06% and 95.55% respectively,The sensitivity,specificity,total consistent rate and positive predictive value of fructosamine were 82.81%,92.50%,88.19% and 89.83% respectively. The result showed that HbAlc was optimal for the diagnosis of GDM and then came to fructosamine. Conclusion It is of great value that HbAle and GSP can be used to diagnose gesrational diabetes mellitus.
