摘要 目的 探讨螺旋CT多期增强扫描测定肾小球滤过率(GFR)的方法 及其准确性.方法 选择30例为临床需要而行腹部CT检查的成年无急性肾功能障碍患者,CT检查包括腹部平扫和三期增强扫描(动脉期和肾实质早期、晚期分别于注射对比剂后28s和61 s、94s时开始扫描),对比剂为碘海醇(300mgl/ml),注射总量为90ml,注射速率为3ml/s,并应用Patlak方程计算GFR,同时计算患者肾实质早期CT清除率(28~61S)和晚期CT清除率(28~94s);另外,以半胱氨酸蛋白酶抑制物C(CystatinC)测定所得的GFR作为对照指标.结果 肾实质早期和晚期CT清除率与血清Cystatin C测定的GFR均有相关性(r=0.76、0.73,均P〈0.01),但以肾实质早期CT清除率与Cystatin C测定的GFR相关性最高.结论 螺旋CT多期增强扫描可准确测定GFR. Objective To evaluate the application of multiphasic helical CT scan in measuring glomerular filtration rate (GFR). Methods Thirty adult patients without acute renal disorder underwent CT scan for clinical indications. The CT protocol consisted of an unenhanced scan and three contrast-enhanced scans at 28,61 ,and 94 sec after starting an injection of 90 ml of iohexol (300mg/ml, 3 ml/sec); all scans included both kidneys. Glomerular filtration rate was calculated with a Patlak plot technique; and cystatin C was determined in blood specimens as a reference. Results The early CT clearance was calculated at 28-61 sec after contrast injection and the late CT clearance at 28-94 sec after injection. Both the early CT clearance and late CT clearance had positive linear correlation with cystatin C(r=0.76, P〈0.01 ) and r=0.73(P〈0.01), respectively. Conclusion Glomerular filtration rate can be measured accurately with multiphasic CT scan for patients without acute renal disorder.
