膝关节负重屈曲45°位X线检查的应用价值 被引量:3

摘要 目的:探讨膝关节负重屈曲45°位X线检查诊断膝关节骨性关节炎的价值。方法:2006年3月~2009年2月,63例(92膝)骨性关节炎患者与同期的41例(82膝)正常对照均行负重伸直和负重屈曲45°位的X线检查。应用患者及正常对照者的X线片测量关节间隙,并进行统计分析。根据软骨病变程度,将病例组分为0~Ⅱ组和Ⅲ~IV组进行比较。结果:病例组关节间隙显著小于对照组(P<0.05),以内侧更为明显。病例组患者屈曲45°位内外侧关节间隙均显著小于伸直位(P<0.05),以外侧更为明显。Ⅲ~IV组伸直位和屈曲45°位时内外侧关节间隙均较0~Ⅱ组显著变小(均P<0.05),其中屈曲位时外侧间隙减小最为明显。结论:膝关节负重屈曲45°位X线检查对关节软骨损伤部位及程度有较好诊断意义。 Objective:To investigate the application value of 45 degree flexion weight-bearing radiograph of the knee in the diagnosis of knee osteoarthritis.Methods:Sixty three patients(92 knees) received the X-ray examination with extend weight-bearing position and flexion-45 degree weight-bearing position,and 41 normal people(82 knees)as control group.We measured the cartilage space on X-ray film and the statistic analysis of them was done.The patients were divided into two groups according to the degree of degeneration of the cartilage:0-II group and III-IV group.Results:The cartilage space of the patients group was significantly less than that of the control group(P0.05).The bilateral cartilage space with flexion-45 degree weight-bearing position was significantly less than that in extend weight-bearing position(P0.05).The bilateral cartilage space in the III-IV group was significantly less than that of the 0-II group in the two positions(both P0.05).Conclusion:The X-ray of the knee flexion-45 degree weight-bearing position has a clinical significance to asses the degeneration degree of cartilage.
