摘要
目的探讨呼吸科铜绿假单胞菌（PAE）感染的相关因素,预防与控制PAE医院感染的发生。方法对2008年6月~2009年3月呼吸科PAE感染病例进行目标监测,并对同期环境（医务人员手、医疗设备及环境表面）进行动态卫生学监测。结果657例呼吸科患者发生医院感染74例,其中PAE感染29例,占39.19%,主要是下呼吸道PAE感染;352份临床标本中分离出PAE 71株,痰标本PAE分离率最高（65株）,其中检出1株对5种抗菌药物耐药的多药耐药铜绿假单胞菌（MDRPA）;345份环境监测标本检出PAE 17株,以水龙头、使用中呼吸机管道、医务人员手PAE检出率高;PAE对抗菌药物耐药率〉50.00%是头孢曲松（76.06%）、头孢噻肟（54.93%）,对亚胺培南耐药率为9.86%;PAE感染与抗菌药物大量、长期应用、高龄、曾入住呼吸科重症监护病房、机械通气、环境污染、无菌操作不规范等有关。结论加强PAE感染病例监测,合理使用抗菌药物,全面实施有效护理干预,对预防和控制PAE感染的发生至关重要。
OBJECTIVE To prevent and control the occurrence of Pseudomonas aeruginosa（PAE） nosocomial infection,and to investigated the relevant factors of PAE infection in the department of respiratory medicine. METHODS The PAE infection cases in the department of respiratory medicine,and the Dersonal and environmental hygiene（hands of medical staff,medical equipment and environmental surfaces） from Jun 2008 to Mar 2009 were dynamically monitored.RESULTS Among 657 patients in the department of respiratory medicine,74 cases of nosocomial infection,occurred and PAE infection was found in 29 cases（39.19%）.PAE infected mainly in lower respiratory tract.From 352 clinical samples 71 PAE strains were isolated,and one strain was multidrug-resistant to five kinds of antibiotics PAE（MDRPA）.Of 345 environmental specimens,17 PAE strains were detected,the high detection of rate PAE was on tap,ventilator-pipes,hands of medical staff.The PAE strains were more than 50.00% resistant to ceftriaxone（76.00%） and cefotaxime（54.93%）,but to imipenem was 9.86%.PAE infection was related to substantial long-term application of antibiotics,old age,ICU,mechanical ventilation,environmental pollution,non-standardized asepsis and so on.CONCLUSIONS It must strengthen the monitoring of PAE infection and rational use of antibiotics.The full implementation of an effective nursing intervention is essential for the prevention and control of PAE infection.
