摘要
目的探讨64层螺旋CT三维重建技术在主动脉夹层诊断中的应用价值。方法对23例主动脉夹层患者进行64层螺旋CT平扫加增强扫描,并利用原始数据在AW4.4工作站进行三维重建。结果64层螺旋CT横轴位图像上可观察到真假腔形态、大小、剥脱的内膜瓣片、破口位置及内膜和主动脉壁钙化。三维重建图像能观察主动脉弓受累情况,确定内膜剥离范围,显示内膜与各分支血管之间的关系。按DeBakey分型,表现为Ⅰ型3例,Ⅱ型1例,Ⅲ型19例。结论应用64层螺旋CT扫描及三维重建技术能对主动脉夹层明确诊断并分型,具有准确、快捷、简便及无创的特点,是诊断该病的首选影像学检查方法。
Objective To evaluate the diagnosis of dissecting aneurysm of aorta (ADA) with 6g-slice spiral CT and 3-dimensional reconstruction. Methods 23 patients with ADA underwent plain scan and contrast enhancement scan by 64-slice spiral CT, and all raw data were processed with 3-dimensional reconstruction in the AW4.4 workstation. Results On axial images, the true and false cavity's morphous and range, intimal flap, location of rupture, and calcification of intima and aortic wall were evaluated. Images of 3-dimensional reconstruction showed involvement of ADA aortic arch, determined the range of intimal flap, and displayed the relationship between the intima and branching vessels. According to type of De Bakey, 3 cases were type Ⅰ; 1 case was type Ⅱ; 19 cases were type m. Conclusion (M-slice spiral CT and 3-dimensional reconstruction can unequivocally discriminate the types of ADA and should be the first choice in diagnosis of ADA.
