摘要
目的探讨超声引导下压迫修复联合凝血酶注射治疗假性动脉瘤的临床价值。方法对32例假性动脉瘤患者,在超声引导下压迫封闭瘤腔与载瘤动脉间的通道,同时保证载瘤动脉远端血运通畅。超声监示下使用22G或20G细针徒手穿刺进入假性动脉瘤残腔,注入凝血酶。用二维和彩色多普勒超声观察瘤体内血栓形成情况。血栓充分形成后观察载瘤动脉及其远端动脉,确定其通畅。结果32例假性动脉瘤在超声引导下压迫修复联合凝血酶注射治疗均获成功,封闭成功率达100%,无中转外科治疗患者,无动脉栓塞、过敏反应等并发症发生。其中1例术后第三天复发,再次应用上述方法治疗后痊愈。结论超声引导下压迫修复联合凝血酶注射治疗假性动脉瘤安全、实用,可在临床推广应用。
Objective To evaluate the safety and practicality of ultrasound-guided compression repair (UGCR) and ultrasound-guided thrombin injection (UGTI) in the treatment of pseudoaneurysm. Method Ultrasound-guided compression repair method for closure of aneurysm cavity and contains the channel between the aneurysm, while ensuring smooth blood circulation distal aneurysm containing. Under ultrasound guidance, 22G or the 20G pintle unarmed puncture entered the pseudo aneurism remnant cavity. Then two-dimensional supersonic and colored Doppler observed the tumor thrombus formation in vivo. Thrombus formation observed after the full set aneurysm and its distal arteries to determine its patency. Results 32 patients were treated with UGCR combine with UGTI of pseudoarteurysms. The closure successful rate of surgery was 100%. There were no complications. Conclusion UGCR combine with UGTI of pseudoaneurysms is safe and practical.
