经皮穿刺水刀髓核切吸术治疗腰椎间盘突出症 被引量:4

摘要 目的:评估经皮穿刺椎间盘水刀髓核切吸术治疗腰椎间盘突出症的临床可行性。方法:2009年2月~6月自协作组内选取符合本研究入选标准的腰椎间盘突出症患者69例,采用美国Hydro-Cision公司生产的SpineJet(脊柱椎间盘水刀)在C型臂X线机、CT或DSA引导下行经皮穿刺椎间盘水刀髓核切吸术。观察并记录术后15天、30天、90天、150天、210天和270天的疗效。结果:用改良Macnab方法进行评估,术后15天、30天、90天、150天、210天和270天的疗效优良率分别是56.5%,82.6%,88.4%,98.6%,98.6%,98.6%,治疗效果与术后时间呈正相关(P<0.01)。结论:经皮穿刺椎间盘水刀髓核切吸术具有操作简单、易于掌握、创伤小、不影响脊柱生物力学稳定性、无严重并发症等特点。 Objective:To invnestigate the clinical feasibility of percutaneous hydrodiscectomy on lumbar disc herniation.Methods:from February to June 2009,we performed percutaneous hydrodiscectomy guided by C-arm,CT or DSA using SpineJet manufactured by HydroCision Incorporation on 69 patients diagnosed as lumbar disc herniation selected from coordination center.The therapeutic effects were observed on the 15th,30th,90th,t150th,210th day and 270th day.Results:The excellent or good outcome rate after procedure evaluated with the modified Macnab method was 56.5%,82.6%,88.4%,98.6%,98.6%and 98.6% respectively on the 15th day,the 30th day,the 90th day,the 150th day,the 210th day and the 270th day.The effectiveness and the postoperative time was positively correlated (P0.01).in 270 days after operation.Conclusion:Percutaneous hydrodiscectomy could be used to treat pure herniated lumber disc,which is simple and minimally invasive and does not affect spinal kinetic stability.
