摘要
目的分析老年人胃十二指肠溃疡穿孔的临床特点与外科治疗。方法对67例老年胃十二指肠溃疡穿孔患者的临床资料进行分析。结果67例均行手术治疗,其中行穿孔单纯修补50例,胃大部切除17例。治愈62例,死亡5例(7.46%)。术后并发症16例:肺部感染9例、急性心功能不全2例、中毒性休克3例、切口感染2例。结论老年胃十二指肠溃疡穿孔病情复杂,一经确诊,宜尽早手术,同时应重视围手术期处理和营养支持。
Objective To investigate the clinical feature and surgical treatment of gastroduodenal ulcer perforation in aged patients.Methods The clinical data of 67 aged patients with gastroduodenal ulcer perforation was analyzed.Results All patients received surgical treatment,50 cases underwent simple perforation repair,17 cases underwent subtotal gastrectomy,62 cases were cured,5 cases died（7.46%）.The postoperative complications were found in 16 cases,including 9 cases with pulmonary infection,2 cases with acute cardiac dysfunction,3 cases with toxic shock,2 cases with wound infection.Conclusion The clinical condition of elderly patients with peptic ulcer perforation was complicated,the mortality was relatively high,and once diagnosed,the patients should receive surgery in time.It is important to pay attention to the perioperative managemen and nutritional support.
