摘要
目的分析医院门诊麻醉药品使用情况,为临床合理用药提供参考。方法收集浙江省桐乡市第一人民医院2007年1月至12月的门诊麻醉处方1492张并统计分析。结果该院共使用麻醉药品5种,其中处方数最多但总用药量最少的是枸橼酸芬太尼注射液,主要用于无痛人流手术镇痛;总用药量最高的是盐酸哌替啶注射液,主要用于体内结石引起的内脏绞痛,其次是硫酸吗啡控释片,主要用于癌症止痛,再次是磷酸可待因片,主要用于镇咳;处方数最少的是盐酸吗啡注射液,主要用于急性心肌梗死、急性心力衰竭等引起的心脏绞痛。药物利用指数(DUI)除盐酸哌替啶注射液和盐酸吗啡注射液大于1.0外,其余3种均不大于1.0。结论该院门诊麻醉药品使用情况不够合理,有待完善;应合理使用麻醉药品,减少对肿瘤患者哌替啶的使用。
Objective To investigate the narcotic drugs utilization in the outpatient department of Tongxiang Municipal First People＇s Hospital and to provide reference data for rational clinical use of these drugs. Methods To adopt the methods of prescription investigation, 1 492 prescriptions of narcotic drugs were collected and analysed in the outpatient department from January to December 2007. Results Usually, 5 kinds of narcotic drugs were used. The prescription number of Fentanyi Citrate Injection was maximum, used for analgesia in painless induced abortion. Pethidine Hydrochloride Injection accounted for first place of DDDs, mainly used in viscera colic caused by internal calculi. Morphine Sulfate Controlled-Release Tablets was followed by, mainly used in relieving cancer pain. Then Codeine Phosphate Tablets, mainly used in relieving cough. The prescription number of Morphine Hydrochloride Injection was minimum, mainly used in angina pectoris caused by acute myocardium infarct, acute heart failure, etc. Drug utilization index（DUI） among pethidine hydrochloride injection and morphine hydrochloride injection were greater than 1.0, the remaining 3 were less than 1.0. Conclusion Narcotics utilization in the outpa- tient department of our hospital is not so rational. There are some problems to be solved. Analgesics must be used rationally. Pethidine utilization should be decreased in tumor patients.
