摘要
目的:探讨社区卫生服务中心通过健康档案管理高血压患者的经验。方法:收集17 509份健康档案,从中筛查高血压病患者2 076例(11.86%),分组管理,开展系统的健康教育。结果:高血压的知晓率达70%,治疗率39.1%,控制率10%。结论:社区卫生服务是长期开展高血压等慢性病管理的有效途径,与三级医院共同携手更有利于高血压的防控。
Objective： To research the experience of using health archives to manage hypertension in community hospitals. Methods： Collect 17 509 health archives, then screen all the hypertensive patients from these health archives, and manage them with groups. Results： Through a series of activities, the status of hypertension awareness is improved. Meanwhile, the physician - patient communication is enhanced. The physicians will know about the physical status and health needs, therefore they can make a plan about health education and management measures and lay a good foundation for making scheme that how to treat hypertension logically and canonically .Conclusion： Communication health service is a favorable condition for chronic disease management. If the big hospitals and community hospitals can be banded together, and have complementary advantages, it will be a valid measure to prevent and eontrol hypertension.
