摘要
目的介绍指掌侧推进皮瓣治疗手指屈曲挛缩畸形的新术式。方法临床应用该方法治疗10例15指,均在患指及其近侧手掌设计皮瓣,皮瓣在近侧手掌部位形成V—Y推进成形,手掌皮瓣可随手指伸直推进到指掌侧,血管神经挛缩轻者通过自身牵伸延长增加长度,重者通过切断桥接指掌侧固有动脉神经解决其短缩问题。该组畸形重者远、近指间关节均属屈曲接近90°、皮肤近端推进距离最长达3cm。结果术后皮瓣全部成活,患指均可充分伸直,患指被动屈伸活动好,经康复锻炼或二次手术松解主动活动均恢复满意,指掌侧皮肤质地好,患指皮肤感觉正常。结论该术式方法简单,效果可靠,对于手掌及指掌侧皮肤无瘢痕区或瘢痕区较轻的手指屈曲挛缩畸形是一个良好选择。
Objective To introduce a new modrandi of curing digital flexed contracture malformation palmar digital pushed flaps. Methods To apply 5 cases including 8 fingers, the faps were all designed on the proximal palm. Like V - Y pushed flap, the pahnar flaps could be pushed to digital side following the extention of the fingers,if the contraeture of vessels and nerves is minor, autoextension can extend length,otherwise,the shortening can be solve by bredging the cut - off palmar digital propera. The angle of proximal and digtal interdigital joints of the fingers with most severe flexion was even up to 90° ,the pushed distance of pahnar flap is 3 cm at large. Results All the post - op flaps survived, and the injured fingers could be straightened completely with good passive activity of extension and flexion. Also, the active were satisfied after re- habilitation exercises or the secondary relaxtion surgery. Thus, the texture and aenastion of palmar digital skin were also fine. Conclusion This operation is simple and effective. It is a good choice for digital flexed contracture malformation with minor palmar shin scar.
