摘要
目的分析慢性乙肝患者丙氨酸氨基转移酶（ALT）水平低于80U／L时，患者体内HBV病毒的复制情况，探讨该类患者有无必要进行抗病毒治疗。方法选取179例慢性乙肝患者，分别测定ALT，HBV病毒载量，HBeAg，并按ALT水平分为三组：I组ALT≥80U／L，Ⅱ组40U／L〈ALT〈80U／L，Ⅲ组ALT≤40U／L。结果①ALT≥40U／L的共130例（72．6％），其中≥80U／L的标本53例（29．6％）。②Ⅰ，Ⅱ，Ⅲ组中，HBV—DNA载量均有阳性出现，分别为16．8％（30／179），10％（18／179），1．7％（3／179），说明各组均有患者处于病毒复制阶段。Ⅰ，Ⅱ，Ⅲ组中，均有HBeAg阴性出现，分别为17．3％（31／179），35．2％（63／179），25．7％（46／179）。③随着ALT水平增高，HBV—DNA载量数值增大，病毒复制处于高水平的标本增多。结论慢性乙肝患者ALT水平80U／L时，尤其是HBeAg阴性的患者，仍可能存在病毒复制。在决定抗病毒治疗前应对此类患者给予关注。
Objective To analyze status of HBV replication in chronic hepatitis B patients with less than 80U/L and explore whether the anti-virus therapy was needed for this group of patients. Methods Blood specimens were obtained respectively from 179 patients with chronic HBV infection and were examined ALT,HBV-DNA qualification and HBeAg,and divide these patients into three groups according to the ALT level：Group Ⅰ ALT≥80U/L;Group Ⅱ 40U/L〈（ALT〈80U/L; Group ⅢALT≤40U/L. Results Total 179 cases were included in this study. 130 cases（72.6%） were ALT ≥40U/L, and 53（29.6%） cases were ≥80U/L. All the Group Ⅰ , Ⅱ and Ⅲ include HBV DNA qualification positive sanples,and the rates were 16,8%, 10% and 1.7% respectively,indicating that patients with virus replicating occurs in all these three groups. All three groups had HBeAg negative samples,the rates were 17.3% ,35.2% and 25. 7% respectively. The number of copies of HBV DNA qualification rose along with the increase of the ALT level. Conclusion HBV are still replicating in some patients with ALT less than 80U/L,especially the patients with HBeAg negative result. It is necessary to give attention to them before performing anti-virus treatment.
