摘要
【目的】研究可乐定对阻塞性睡眠呼吸暂停低通气综合征(OSAHS)患者行悬雍垂腭咽戍形术(UPPP)术后的镇静作用。【方法】60例择期全身麻醉下行UPPP患者,术毕在麻醉恢复室观察治疗。随机均分为:可乐定组和对照纽。可乐定组给予可乐定3μg/kg,对照组给予等量生理盐水。观察两纽患者在入恢复室时(T1),术后2h(T2),术后4h(T3),术后8h(T4),拔管前3h调整镇静镇痛药物用量时(T5),拔管前1h停用镇静镇痛药物时(T6)以及拔管前即刻(T7)的血压、心率、镇静评分(sedation agitation scale,SAS评分)和脑电双频指数,记录观察治疗过程中患者出现体动、恶心、呕吐的次数。【结果】两组患者在各时点比较血压、心率无统计学差异(P〉0.05),但血管活性药物用量可乐定组明显少于对照组(P〈0.01)。躁动次数、恶心、呕吐、吞咽活动的发生率可乐定纽明显低于对照组(P〈0.05)。3μg/kg可乐定静注对BIS无明显影响。【结论】静注可乐定3μg/kg可以增加患者对于气管插管的耐受性,明显减少恶心、呕吐、体动等不良事件的发生,但对镇静评分及BIS无明显影响。
[Objective] To investigate the effect of clonidine in the sedation management of postoperative patients with severe obstructive sleep apnea hypopnea syndrome(OSAHS) undergoing uvulopalatopharyngoplasty(Uppp) surgery. [Methods] Sixty patients who suffered from severe OSAHS, and underwent elective UPPP surgery, and were monitored in PACU overnight after operation were randomly divided into control group ( n =30) and clonidine group ( n =30). Patients in elonidine group received clonidine 3 μg/kg(diluted to 5 mL) with slowly intravenous injection(in 3 minutes). Patients in control group received normal saline in same volume. The blood pressure(BP), heart rate(HR), SPO2 and bispeetral index(BIS) were recorded at the time entering PACU, 2 hours, 4 hours, 8 hours after entering PACU, 3 hours, 1 hour and instantly before discharge from extubation. The incidence of agitation, nausea and vomit was recorded. [Results] There were no difference in BP and HR between two groups, but the administration of vasoactive agents in clonidine group was less than that in control group ( P 〈0.01). The incidence of restlessness, nausea, vomiting and swallow in clonidine group was less than that in control group ( P 〈0.05). There was no significant correlation between BIS and clonidine. [Conclusion] Intravenous clonidine 3ug/kg can increase the tolerance of the patients to the tracheal intubation, and reduce the incidence of the complications such as restlessness, nausea and vomiting, but it can not affect BIS and sedation score.
