新诊断EH与胰岛素抵抗及糖脂代谢异常相关性研究 被引量:1

Study on IR and Glucose Lipid Metabolic Abnormality in Newly-diagnosed EH

摘要 目的探讨新诊断原发性高血压(EH)与胰岛素抵抗及糖脂代谢异常的相关性,以期临床上予以关注,进行适当的早期干预治疗。方法将62例EH分Ⅰ级EH组和Ⅱ级EH组,根据空腹血糖、胰岛素、HOHA-IR、ISI及TC、TG分别进行此两组间及对照组比较。结果Ⅱ级EH组FPG、FINS、HOMA-IR增加均大于Ⅰ级EH组及对照组(P<0.01)。ISI:Ⅱ级EH组小于Ⅰ级EH组和对照组(P<0.05)。Ⅰ级EH组中IFG发生率为78.1%;Ⅱ级EH组中IFG发生率为43.3%,DM发生率为56.7%。Ⅰ级EH组中高TC、高TG发生率分别为31.3%和37.5%,Ⅱ级EH组中高TC、高TG发生率分别为53.3%和63.3%(组间分别比较均P<0.01)。结论新诊断原发性高血压与胰岛素抵抗及糖脂代谢异常有相关性,Ⅱ级EH组较Ⅰ级EH组显著。 Objective To investigate insulin resistance and glucose lipid metabolic abnormality in different grades patients with newly-diagnosed essential hypertension. Methods 62 EH patients were enrolled. Based on grade of hypertension, patients were divided as grade Ⅰ group and grade Ⅱ group, the FPG, TINS, HOMA-IR, ISI and TC, TG were determined and analyzed. Resuits Compared with control group and grade Ⅰ group, FPG,FINS, HOMA-IR were progressively aggravated in Ⅱ grade group (P 〈0.01 ). Patients in grade Ⅱ group had most seriously impairment of ISI vs grade I group (P 〈0.05). The incidence rate of IFG was 78.1% in grade Ⅰ group and the Ⅱ grade EH group was 43.3% (P 〈0.01 ). In grade Ⅱ group, the incidence rate of DM was 56.7%, TC, TG were higher in grade I group (53.3% vs 31.3% and 63.3% vs 37.5% ) (P 〈 0.01 ). Conclusion Our data showed that newly-diagnosed EH was related to IR and glucose lipid metabolic abnormality.
