颅脑外伤患者心肌损害的相关性研究

摘要 目的探讨不同程度颅脑外伤患者动态心电图、心肌酶谱及血清肌钙蛋白I(CTnI)的变化,了解急性颅脑损伤所致心肌损害的临床表现特点。方法185例急性颅脑外伤患者,根据Glasgow昏迷评分分为轻型组(37例),中型组(58例)和重型组(90例),全部病例在入院后立即进行动态ECG检查,1周后复查,并于入院即刻检测心肌酶谱及血清肌钙蛋白I。结果急性颅脑外伤患者伤后出现多种心电图异常,发生率与病情严重程度相关,总发生率为78.4%,在入院24h内表现显著,之后随着时间推移有所改善(1周后为38%);急性颅脑损伤患者伤后血CK、CK-MB、LDH、HBDH及CTnI水平显著升高,且与患者病情轻重程度密切相关。结论急性颅脑外伤患者存在不同程度的心肌损伤,且与其损伤程度密切相关。 Objective To study the alterations of dynamic ECG, various indexs of myocardial enzymology and CTnI in different kinds of brain injury patient. Methods A total of 185 patients with acute brain injury accepted dynamic ECG monitoring immediately and one week after admission, respectively. All of them underwent blood biochemical tests immediately after admission. Results Muhiple anomalous ECG were found in 78.4% acute brain injury patients and were obvious abnormal within 24 halter admission. The level of blood CK,CK-MB,LDH,HBDH and CTnI was significantly increased after acute brain injury and was closely related to the damage degree of the injury condition. Conclusion There were different degree of myocardial damage in patients with acute brain injury and the myocardial damage maybe closely correlated with thedegree of traumatic brain injury
