FiO2×PEEP对重症甲型H1N1流感致急性肺损伤病情评估与预后价值研究 被引量:7

摘要 目的观察重症甲型H1N1流感导致急性肺损伤(ALI)患者FiO2×PEEP水平的变化,探讨其对最症甲型H1N1流感导致ALI严重程度及预后评估的价值。方法对2009—10—28~2009—12—28我院EICU收治的10例(存活组6例,死亡组4例)重症甲型经H1N1病毒感染患者进行病例回顾分析,分别监测每组患者每天的FiO2、PEEP、PaO2,并计算FiO2×PEEP及氧合指数(PaO2/FiO2),比较FiO2×PEEP与氧合指数之间关系,动态比较两组治疗前后FiO2×PEEP的变化趋势及其差异。结果①FiO2×PEEP与氧合指数呈负相关(r=-0.44,P〈0.05);②存活组治疗前后FiO2×PEEP均低于死亡组;③存活组在治疗干预后FiO2×PEEP逐渐下降,而死亡组FiO2×PEEP无下降趋势。结论FiO2×PEEP对重症甲型H1N1流感病毒感染导致急性肺损伤病情评估及预后判断具有良好的临床价值. Objective To investigate the prognostic value of FiO2 × PEEP in acute lung injury caused by severe influenza virus A (H1N1). Methods 10 patients with acute lung injury, caused by severe HINI in our emergency department were divided into the survival group (n = 6) and the death group (n =4) between October 28, 2009 and December 28, 2009, the parameters of FiO2, PEEP, PaO, were recorded daily and the FiO2 × PEEP and oxygenation index were calculated. The relationship between FiO2 × PEEP and oxygenation index was analysed, the dynamic changes of FiO2 × PEEP before and after the treatment and the differences of FiO2 × PEEP between the two groups were compared. Results (1) The parameter of FiO2 ×PEEP was negatively correlated with oxygenation index(OI) ( r = -0.44,P 〈0.05 ) ; 2) The parameter of FiO2 x PEEP in the survival group was obviously lower than that in the dead group; 2)FiO2 × PEEP gradually decreased in the survival group after the treatment, while tbere was no obvious changes in the dead group. Conclusion The parameter of FiO2 × PEEP has good clinical assessment and prognostic value in acute lung injury caused by severe influenza virus A (H1N1).
