摘要
目的:探讨结直肠癌合并肺部结节的手术疗效。方法:统计26例结直肠癌合并肺部结节的患者接受手术治疗的资料,随访并分析其生存情况。结果:所有患者手术顺利,无术中术后死亡。1例吻合口瘘经过常规处理2周后顺利愈合。术后病检11例肺部结节为良性,15例为肠道肿瘤的远处转移。25例患者获得随访,平均生存时间43.5月,中位生存时间40.0月。按肺部结节性质分为肺部转移组和非转移组,非转移组平均生存时间48.9月,中位生存时间49.0月;转移组平均生存时间36.0月,中位生存时间37.0月。两组生存曲线比较,非转移组好于转移组,但差异无统计学意义(P=0.060)。结论:合并肺部结节的结直肠癌患者,行原发灶及肺部结节同时切除治疗,其手术安全、可行,且能够获得较满意的疗效。
Objective： To investigate the effect of surgical treatment in colorectal carcinoma combined with lung nodule. Metholds：Collected the clinical data with surgical treatment in 26 colorectal carcinoma cases combined with lung nodule. Results：All operations were successful,without death in operation and postoperation. One case with stoma fistula smoothly healed after two weeks of conventional treatment. Fifteen cases were diagnosed as lung metastasis from intestinal tumor by pathology. Average survival time（AST） was 43.5 months, median survival time（MST） was 40.0 months, for 25 cases with complete follow -up. The cases were divided into lung metastasis group （LMG） and nonlung metastasis group（NLMG）. The AST and MST were 48.9 months and 49.0 months respectively in NLMG, and were 36.0 months and 37.0 mongths respectively in LMG. It was better in NLMG than LMG, but the difference was not statistically significant（P = 0. 060）. Conclusion： Resection of both colorectal carcinoma and lung nodule is reliable and feasible,it has satisfactory effect.
