摘要
目的分析院外急救中影响低血糖昏迷患者治疗效果的多种因素。方法138例低血糖昏迷患者，开始治疗前取外周血测血糖。静脉给予葡萄糖，使血糖维持在正常范同。经院外治疗在到达医院时已转醒为显效（显效组），到达医院时昏迷程度好转为有效（有效组），到达医院时昏迷程度无变化为无效（无效组）。分析影响治疗效果的因素。结果显效41例，有效35例，无效62例，院外急救治疗有效率为55．1％。显效组、有效组年龄均显著低于无效组（均P〈0．05），显效组病程时间和昏迷时间显著短于无效组（均P〈0．01），血糖浓度显著高于无效组（均P〈0．01），有效组昏迷时间显著低于无效组（P〈0．05）。组间两两比较显永：昏迷程度越深，总有效率差越低（均P〈0．01）。结论年龄、病程时间、昏迷时间、昏迷程度、血糖浓度等因素影响着院外急救中低血糖昏迷患者的治疗效果。
Objective To investigate the factors influencing curative the effects in patients with hypoglycemic coma during pre-hospital medical rescue. Methods 138 patients with hypoglycemic coma underwent pre-hospital first aid with intravenous treatment of glucose. Before the treatment peripheral blood samples were collected to examine the blood sugar concentration. The relations of curative effect to different factors were analyzed. Results When the patients arrived at the hospital, 41 of them had come to （obviously effective group）, 35 had their coma degrees becoming lighter （partial effective group）, and 62 showed no change in their coma degrees （ineffective group）. The total rate of effects （obvious and partial effects） after the pre-hospital emergency treatment was 55.1%. The average ages of the obviously effective and partial effective groups were both significantly lower than that of the ineffective group （both P〈0.05）. The course time and coma time of the obvious effective group were significantly shorter, and the blood sugar concentration of the obvious effective group was significantly higher than that of the ineffective group （all P〈 0.01）. And the coma time of the partial effective group was significantly shorter than that of the ineffective group （P〈0.05）. The analysis between any 2 groups showed that the deeper the coma degree the lower the total effective rate （all P〈0.01）. Conclusion Age, course time, coma time, coma degree, and blood glucose concentration affect the treatment effect in the patients with hypoglycemic coma during pre-hospital first aid.
