摘要
[目的]了解太原市肺结核患者拒绝免费治疗情况及原因,为进一步规范结核病管理工作提供依据。[方法]2009年2月,对太原市拒绝免费治疗的378例病人进行问卷调查。[结果]调查的378例拒绝免费治疗肺结核患者中,初中以下文化程度占44.44%;农民(农民工)占24.07%,工人占21.69%;涂阳病例占38.89%;初诊单位在综合医院的占66.67%;确诊单位在综合医院、结核病专科医院、结核病防治机构的分别占32.01%、50.27%、17.73%。有367例接受治疗,其中在结核病专科医院治疗的占65.40%,在综合医院治疗的占28.89%;医生嘱咐治疗时间不统一,不知道停药时间的占14.17%。拒绝免费治疗的原因主要是比较信任专科医院(占44.69%),听医生说免费药品不好(占29.43%)。[结论]太原市肺结核患者拒绝免费治疗者所占比例较高,主要原因是比较信任专科医院、听医生说免费药品不好。今后需加强对结核病免费治疗政策的宣传力度。
[Objective]To understand the reason that TB patients refused to free treatment in Taiyuan city,to provide evidence for standardizing TB management. [Methods]Questionnaire was conducted for 378 tuberculosis patients who refused free treatment in Taiyuan in February 2009. [Results]378 TB patients who refused free treatment were investigated, of them the patients with the educational level of junior high school and below accounted for 44.44% ,farmers and workers accounted for 24. 07 % and 21.69 %,smear positive patients accounted for 38.89 %. The patients who first visited general hospital accounted for 66.67 %. The diagnosis confirmed in general hospital,TB hospital and Institute for TB Prevention and Control accounted for 32.01% ,50.27% and 17.73% respectively. Of the 367 patients received treatment 65. 40% them were treated in TB hospital and 28.89% of them were treated in general hospital. The reason that the visiting time given by the doctors was not the same and no clear withdrawal time accounted for 14.17 %. The main reason that the patients refused free treatment was trusting special hospital （44.69%） and the doctor that the drugs free of charge was no curative effect（29.43 %）. [Conclusion]The proportion of the patients who refused free treatment was higher in Taiyuan city. The main reason that the patients refused free treatment was trusting special hospital and the doctor that the drugs free of charge was no curative effect. Propaganda on the policy of TB free treatment must be strengthened.
