摘要
目的:研究羟丁酸钠对学龄前儿童静脉滴注氯胺酮药代动力学的影响。方法:16例3.5~6岁行择期手术的学龄前儿童随机分为两组:Ⅰ组为氯胺酮组,Ⅱ组为氯胺酮+羟丁酸钠,两组均于气管插管后静脉滴注0.05%氯胺酮3mg·kg^(-1)·h^(-1),用反向高效液相色谱法测定氯胺酮血药浓度,采用中国药理学会编辑的3P_(87)软件计算药代动力学参数。结果:两组房室模型均拟合为二房室开放模型。药代动力学参数;Ⅰ组T_(1/2)α=0.15±0.04h,T_(1/2)β=2.84±0.44h,Cl=0.34±0.06L·kg^(-1)·h^(-1),Vd=0.66±0.29L·kg^(-1)·h^(-1);Ⅱ组T_(1/2)α=0.15±0.04h,T_(1/2)β=2.48±0.45h,Cl=0.43±0.09L·kg^(-1)·h^(-1),Vd=0.64±0.12L·kg^(-1)。结论:羟丁酸钠使氯胺酮清除半衰期缩短,清除率增加。
Objective: To determine the influence of γ-hydroxybutyric sodium (7-OH)on the pharmacokinetics of ketamine infusion in preschool children. Method: Sixteen patients were randomly assigned to group Ⅱ (Ketamine) and group Ⅱ (Ketamine plus γ-OH). After trachea intubation,0.05% ketamine was infused in both groups at 3mg·kg^(-1)·h^(-1) and γ-OH 125mg·kg^(-1) only in group Ⅱ. The plasma concentration of ketamine was measured using high performance liquid chromatography. Pharmacokinetic parameters were calculated using 3P_(87) software composed by Chinese Pharmacologic Society. Result: In both groups,pharmaeokinetics could be descrided as two-compartment open model. The T_(1/2)β and the Cl differ between group Ⅰ and group Ⅱ (P<0.01). Group Ⅰ: T_(1/2α)=0.15±0.04h, T_(1/2)β=2.84±0.44h,Cl=0.34±0.06L·kg^(-1)·h^(-1),Vd=0.66±0.29L·kg^(-1)·h^(-1);Group Ⅱ: T_(1/2α)=0.15±0.04h. T_(1/2)β=2.48±0.45h,Cl=0.43±0.09L·kg^(-1)·h^(-1), Vd=0.64±0.12L·kg^(-1). Conclusion: γ-OH can shorten the half-life of ketamine and promote its clearance.
