摘要
目的研究我国血液透析患者人群中经输血传播病毒(TTV)的感染及基因变异状况。方法设计特异性引物采用半巢式 PCR 方法对北京市2所综合性大医院50例血液透析患者血清标本进行 TTV DNA 的检测并将产物克隆及序列分析。结果经过半巢式 PCR 检测,血透患者 TTV DNA 阳性率为14%。对其中4例标本中分离出的TTV 270个碱基核酸片段序列分析表明:基因同源性为99.1%~100%,推测的编码氨基酸序列同源性为100%。与国外报道的日本株比较,核苷酸同源性为88.3%~97.8%,推测的编码氨基酸同源性为96.1%~98.7%。TTV 阳性与阴性组转氨酶升高的患者比例差异无显著性。结论我国血透患者中存在较严重的 TTV 感染,其病毒部分基因及推测编码蛋白十分保守,但与日本所报道的 TTV 毒株有明显差别。未见 TTV 感染与转氨酶升高密切相关的证据。
Objective To study the infection and genetic variation of transfusion transmitted virus(TTV)in hemodialysis patients.Methods 50 hemodialysis patients from Beijing were studied. DNAs of TTV were amplified by semi-nested polymerase chain reaction(PCR)with specific primers. The PCR products were cloned and sequenced.Results 14% of hemodialysis patients were positive for TTV using semi-nested PCR.270bp fragment of TTV DNA were isolated from 4 patients.Among 4 isolates of TTV(T001-T004)the gene sequence homologies were above 99.1% and the deduced amino acid sequence homologies were 100%.The comparison of these 4 isolates and 4 Japanese isolates reported previously showed that the gene sequence homologies were in the range from 88.3% to 97.8% and deduced amino acid sequence homologies were from 96.1% to 98.7%.There were not significant differences in the prevalence of transaminase(ALT)elevated between hemodialysis patients with positive and negative of TTV marker.Conclusion TTV infection is found in China.The gene and deduced amino acid sequences of TTV are highly conserved among isolates in this study.
