摘要
目的:了解老年住院病人肾功能状况及特点,分析肾功能下降的影响因素。方法:以简化的MDRD公式计算估计肾小球滤过率(estimated GFR,eGFR)的方法,对2006年1月至2008年5月在我院内科系统的464例≥60岁住院患者肾功能进行评估,分析肾功能下降的影响因素。结果:①本组患者有肾脏基础疾病患病率为10.6%,高血压60.8%,糖尿病22.4%,冠心病64.5%,脑血管意外47.8%,检出颈动脉粥样硬化斑块15.3%。②肾功能中、重度受损疾病谱eGFR<60ml/min.1.73m2中,其合并症占前三位为:冠心病为103例(72.7%);高血压为96例(67.1%);糖尿病36例(25.2%)。③病因相关Logistic回归分析显示:年龄、尿酸、高血压、冠心病是肾功能中、重度受损的独立危险因素。结论:老年住院病人中肾功能下降随着年龄的增长而增加;年龄、尿酸、高血压、冠心病是肾功能中、重度受损的独立危险因素。
Objective： To investigate the renal function state of senile inpatients, and its affected factor of the declining of renal function. Method： To evaluate the renal function affected factor of the declining of which with 464 senile patients aged over 60 in our internal medicine during Jan. 2006 to Jun. 2008, glomerular filtration rate （GFR） was calculated by the simplified modification of diet in renal disease （ MDRD） study equation. Result： The morbidity of renal underlying disease was 10.6%, 60.8% with hypertension, 22. 4% with diabetes mellitus, 64.5% with coronary heart disease, 47.8% with cerebrovascular accident and 15.3% carotid atherosclerotic . Among moderate and severe renal impairment （eGFR 〈 60ml/min · 1. 73m^2） the first three of the complications were coronary heart disease with 103 cases（72.7%）, hypertension with 96 cases （60.8%） ,and diabetes mellitus with 36 cases（ 22.4% ）. Results of Logistic regression analysis showed that 4 leinds of factors were independent risk factors of moderate and severe renal impairment including aging, uric acid, hypertension and coronary heart disease. Conclusion： The changes of renal function are decreasing with age in senile inpatients . Age , uric acid, hypertension and coronary heart disease are independent risk factors of moderate and severe renal impairment.
