摘要
目的对上海市杨浦区卫生局研制的《城市社区卫生服务机构医疗保险工作考核指标体系》进行实证检验,为考核体系的应用推广提供依据。方法运用原有考核体系和本考核体系对杨浦区8家社区卫生服务机构2007年1—10月的医保工作进行考核,将考核结果作为检验资料;随机选取其他城区的11家社区卫生服务机构,运用本考核体系对其2007年1—6月与2008年1—6月的医保工作进行考核,将考核结果作为实证检验资料,对考核结果进行两变量的相关分析和成对样本t检验。结果原有考核体系结果与本考核体系结果的相关性有统计学意义(r=0.761,P=0.028);原有考核体系结果的均值为(92.81±4.36)分,本考核体系结果的均值为(77.19±12.74)分,两者比较差异有统计学意义(t=4.492,P=0.003)。2007年1—6月考核结果的均值为(90.09±2.94)分,2008年1—6月考核结果的均值为(93.86±2.73)分,两者比较差异有统计学意义(t=-3.166,P=0.010)。结论本考核体系具有较强的科学性、实用性、可操作性和有效性,适合在医疗保险监管部门对社区卫生服务机构的管理中应用和推广。
Objective To test the ＂ Urban Community Health Service Health Insurance Evaluation System＂ organized and formulated by Shanghai Yangpu District Health Bureau, in order to provide the basis for popularization of the evaluation system. Methods The medical insurance work of the 8 community health service （CHS） institutions in Yangpu district, from January to October 2007 was assessed by using the original and the new systems respectively. The assessed results were used as the test materials. Then 11 CHS institutions in other districs were at random selected, and their insurance work during the periods between January and June of both 2007 and 2008 was assessed, and the assessed results were taken as the positive test materials. Finally correlation analysis of the two variables was conducted for the assessed results, and t test for paired - samples was also conducted. Results The results of two evaluation systems were significantly correlated （ r = 0. 761, P = 0. 028 ）. The average value of assessed result was （92. 81 ±4. 36） for original evaluation system and （77. 19 ± 12. 74） for the new evaluation system, with a significant difference （t = 4. 492, P = 0. 003）. The average value of assessed result was （90. 09 ± 2.94） for the period from January to June 2007 （93.86 ±2. 73 ） for the period from January to June 2008 （t = - 3. 166, P =0. 010）. Conclusion This evaluation system has a strong scientificness, feasibility, operability and effectiveness, it will be suitable for medical insurance regulatory departments to apply and popularize in management of the community health service institutions.
