摘要
目的比较LASIK与LASEK治疗高度、超高度近视的临床疗效。方法选择接受准分子激光近视矫正术(LASEK或LASIK)的患者68例(125眼),其中LASEK组39例(72眼),LASIK组29例(53眼)。入选患者术前等效球镜值(spherical equivalent,SE)为-6.00～-12.00D,平均(-8.91±2.12)D。检查并比较2组患者术后裸眼视力(uncorrected visual acuity,UC-VA)、最佳矫正视力(best corrected visual acuity,BCVA)、SE、角膜上皮下雾状混浊(haze)程度及波前像差和并发症等。结果术后1周时,LASEK组UCVA为4.80±0.11,明显低于LASIK组的5.03±0.13,差异有显著统计学意义(P<0.01);术后1个月、3个月、6个月时,LASEK组UCVA分别为4.96±0.09、5.05±0.22、4.93±0.07,LASIK组分别为5.03±0.13、5.06±0.12、5.02±0.13,2组间比较差异均无统计学意义(均为P>0.05)。术后6个月时,2组BCVA与术前相比不变或有提高,无一例下降。LASIK组未出现haze,而LASEK组术后可见少量haze,无2级以上haze出现。术后1周及1个月,2组患者屈光度均有轻度的过矫状态,2组差异无统计学意义(P>0.05);术后6个月,2组中均有部分患者出现不同程度的屈光回退现象,LASEK组回退至(-0.38±0.69)D,高于LASIK组的(0.01±0.37)D,2组屈光度比较差异有统计学意义(P<0.05)。术后1个月、3个月和6个月时,2组总高阶像差、垂直慧差、水平慧差及球差的均方根值均较术前增加,差异均有统计学意义(均为P<0.05),LASEK组总高阶像差增加倍数均低于LASIK组(均为P<0.05)。结论LASEK矫正高度、超高度近视近期在安全性和有效性方面与LASIK相同,远期疗效有待进一步观察。LASEK治疗高度、超高度近视视觉质量优于LASIK。
Objective To compare the clinical efficacy between laser in situ keratomileusis（LASIK）and laser subepithelial keratomileusis（LASEK）for high and ultra-high myopia.Methods A total of 68 patients（125 eyes）,who intended to take corneal refractive surgery（LASIK and LASEK ）,were included in the study,29 patients（53 eyes） in LASIK group and 39 patients（72 eyes） in LASEK group.Preoperative spherical equivalent（SE） of patients was -6.00 D to -12.00 D,averaged in（-8.91±2.12）D.The uncorrected visual acuity（UCVA）,best corrected visual acuity（BCVA）,SE,degree of haze,high wavefront aberrations and complications were examined and compared.Results At postoperative 1 week,the UCVA was 4.80±0.11 in LASEK group,which was significantly lower than 5.03±0.13 in LASIK group（P〈0.01）.At postoperative 1 month,3 months and 6 months,UCVA were 4.96±0.09,5.05±0.22 and 4.93±0.07 in LASEK group,were 5.03±0.13,5.06±0.12 and 5.02±0.13 in LASIK group,and there were no statistical difference between the two groups（all P〈0.05）.At postoperative 6 months,BCVA of two groups unchanged or improved compared with that preoperatively,and there was no decline in any cases.Haze did not found in LASIK group.Mild haze was found in LASEK group,but the haze was not higher than degree Ⅱ.At postoperative 1 week and 1 month,there were some minor regression occurred in both two groups,no statistical difference was found（P〈0.05）.At postoperative 6 months,there were some refractive regression in both two groups.The regressive degree （-0.38±0.69）D in LASEK group was significantly higher than （0.01±0.37）D in LASIK group（P〈0.05）.At postoperative 1 month,3 months and 6 months,the root mean square value of higher order aberrations,vertical coma,level of coma and spherical aberration were higher than those preoperatively in both two groups（all P〉0.05）.At postoperative 1 month,3 months and 6 months,the increased degree of higher order aberrations in LASEK group was lower than that in LA
