mf-ERG二阶kernel反应在亚临床期糖尿病视网膜病变中的变化 被引量:3

摘要 目的探讨多焦视网膜电图(multifocal electroretinogram,mf-ERG)二阶kernel反应在亚临床期糖尿病视网膜病变的变化特点,评价其发现糖尿病早期视功能变化的作用。方法选择确诊为2型糖尿病、经散瞳眼底镜检查均未见视网膜病变患者32例,进行mf-ERG二阶kernel反应检查,并与正常组对照。结果mf-ERG二阶kernel反应在亚临床期糖尿病视网膜病变中已有异常:在5个环中,1-2环a波潜伏期延迟,1-5环b波潜伏期延迟,1-3环b波振幅总和、振幅密度减低,5环b波振幅密度减低;在4个象限中,鼻上象限b波振幅密度减低,颞下象限b波潜伏期延迟,振幅总和减低,颞上象限a波、b波潜伏期延迟,b波振幅总和、振幅密度减低。结论mf-ERG二阶kernel反应可较敏感地检测出亚临床期糖尿病视网膜病变的视网膜功能异常,具有临床参考价值。 Objective To investigate the changes of second order kernel(SOK) of multifocal electroretinogram(mf-ERG) in detecting the early retinal abnormalities in diabetic retinopathy at sub-clinical stage,and to explore its effects on discovering the visual function changes of diabetes mellitus(DM) at early stage.Methods A total of 32 patients with type 2 DM,which were detected without retinopathy through mydriasis under funduscope,were examined by SOK of mf-ERG and compared with normal control group.Results SOK of mf-ERG had been abnormal in diabetic retinopathy at sub-clinical stage.In 5 rings,the latencies of a-wave were delayed obviously in ring 1 to ring 2.The latencies of b-wave were delayed obviously in ring 1 to ring 5.The sum amplitudes and amplitude densities of b-wave were reduced significantly in ring 1 to ring 3 and the amplitude density in ring 5 of b-wave was reduced too.In 4 quadrants,amplitude density of b-wave in superior-nasal quadrant was decreased,latency of b-wave in inferior-temporal quadrant was delayed,and sum amplitude was reduced;Latencies of a-wave and b-wave in superior-temporal quadrant were delayed;The sum amplitude and amplitude density of b-wave were decreased.Conclusion SOK of mf-ERG is sensitive to detect retinal dysfunction in diabetic retinopathy at sub-clinical stage,and it has the clinical value.
