摘要 目的观察无创正压通气对NYHA 4级心力衰竭患者脑钠肽(BNP)水平的影响。方法选择慢性充血性心衰NYHA4级患者46例,按随机数字表法分成两组,每组各23例,分别给予吸氧+药物治疗或者无创正压通气+药物治疗,检测所有患者治疗前和治疗24h后的BNP水平。结果(1)经治疗后,两组患者BNP水平均下降(P均<0.05)。(2)与常规吸氧组相比,无创正压通气组BNP水平均下降更加显著,两组间比较有统计学差异(P<0.05)。结论短期应用无创正压通气治疗能降低慢性充血性心衰NYHA4级患者的BNP水平。 Objective To assess the effects of noninvasive positive pressure ventilation on the plasma concentration of brain natriuretie peptide(BNP) in patients with NYHA 4 heart failure. Methods A total of 46 patients with NYHA 4 heart failure were randomly divided into two groups with 23 for each: noninvasive positive pressure ventilation group and oxygen uptake group. The oxygen uptake group was given routine therapy and oxygen uptake,and the noninvasive positive pressure ventilation group was given routine therapy and noninvasive positive pressure ventilation. BNP was measured before and after 24h therapy in the two groups, Results (1)BNP was decreased after 24h therapy in the two groups(P〈0.05). (2)Compared with the oxygen uptake group,BNP was lower in the noninvasive positive pressure ventilation group after 24h therapy(P〈0.05). Conclusion Noninvasive positive pressure ventilation can decrease the plasma BNP level in patients with NYHA 4 heart failure.
