摘要
目的探讨与研究螺旋CT灌注成像对肺结核病变的诊断价值。方法选择肺结核患者25例,对比观察肺恶性肿瘤患者25例,行螺旋CT灌注扫描,计算肺部病灶的灌注量(perfusion volume,PV)、血容量(blood volume,BV)、增强峰值(peak height,PH)和对比剂的平均通过时间(mean transit time,MTT)的数值。结果结核PV、BV、PH均值分别为12.15±4.12 ml.min-1.ml-1、10.36±5.87 ml/100 g、18.21±5.58 HU。恶性肿瘤PV、BV、PH均值分别为37.33±15.22ml.min-1.ml-1、22.34±9.36 ml/100 g、38.05±13.21 HU。二者PV、BV、PH均值比较,结核明显低于恶性肿瘤,具有统计学意义(P<0.05)。二者MTT均值分别为30.33±14.36 s、26.36±13.53 s,无明显差异(P=0.917)。结论螺旋CT灌注成像可作为肺结核病变诊断和鉴别诊断的辅助方法。
Objective To evaluate the value of spiral CT perfusion(CTP) in the diagnosis of pulmonary tuberculosis.Methods Spiral CTP was performed in 25 patients with pulmonary tuberculosis and in 25 patients with malignant pulmonary tumors.The parameters of CTPinclude perfusion volume(PV),blood volume(BV),peak height(PH) and mean transit time(MTT) were analyzed.Results The values of PV,BV and PH were significantly lower in pulmonary tuberculosis than in malignant pulmonary tumors [(12.15±4.12) ml.min-1·ml-1 vs(37.33±15.22) ml.min-1·ml-1,(10.36±5.87ml/100 g) vs(22.34±9.36ml/100 g),(18.21±5.58) Hu vs(38.05±13.21) Hu,P〈0.05].However,the differences of MTT between pulmonary tuberculosis and malignant pulmonary tumors were not significant [(30.33±18.36)s,(26.36±20.53)s,P=0.937].Conclusion Spiral CTP is an applicable method in the diagnosis and differentiating diagnosis of pulmonary tuberculosis.
