18G活检抽吸针对肺部小病变及肺门病变穿刺活检的价值 被引量:3

The Value of 18G Biopsy Needles for Smaller Pulmonary Lesions and Hilar Lesions in Percutaneous Lung Biopsy

摘要 目的探讨18G活检抽吸针对肺部小病变及肺门病变穿刺活检的临床应用。资料与方法回顾性分析51例直径<3cm的肺部小病变和肺门病变经CT导引下活检资料,并对其穿刺活检与术后组织病理学结果及临床随访结果进行对照分析。结果51例患者成功获得足够组织标本。其中恶性病变32例,最终确诊30例,准确率93.8%(30/32);良性病变17例,最终确诊14例,准确率82.4%(14/17);未确诊2例。穿刺活检总符合率为90.2%(46/51)。并发症有:气胸4例,占7.8%(4/51),其中1例作微创性胸腔闭式引流;少量咯血2例,占3.9%(2/51);未发现致命性并发症。结论使用18G活检抽吸针对肺部小病变及肺门病变的诊断安全、可靠、准确。 Objective To evaluate the clinical applicationg of 18G biopsy needles for smaller pulmonary lesions and hilar lesions in percutaneous lung biopsy. Materials and Methods CT-guided percutaneous lung biopsy was perfomed in 51 cases of patients that had smaller pulmonary lesions of the diameter ≤3 cm and/or hilar lesions. The biopsy results and postoperative pathological diagnosis or clinical follow-up results were compared. Results 51 patients obtained enough specimens through the punctures. 32 cases of malignant lesions were eventually confirmed 30 cases, and the accuracy rate was 93.8% (30/32). 17 cases of benign lesions were confirmed 14 cases,and the accuracy rate was 82.4% (14/17). The biopsy could not indicate the accurate diagnosis in 2 cases. The total accordance rate was 90.2%. As for complica- tions,4(7.8%, 2/51 )cases suffered pnemothroax, one of them was relieved by closed mini-invasive thoracic drainage. 2 (3.9% ,2/51 )cases experienced mild hemoptysis. No fatal complications were found. Conclusion The application of 18G lung needles to smaller pulmonary lesions and hilar lesions is safe ,reliable and accurate.
