摘要
目的探讨腹壁下动脉穿支皮瓣修复大腿肿瘤切除术后创面的效果。方法自2002年1月至2009年1月，采用腹壁下动脉穿支皮瓣修复大腿肿瘤切除术后创面15例，皮瓣面积13cm×10cm～40cm×10cm。血管蒂长11～16cm。根据缺损区皮肤松弛程度决定采用直接缝合或皮瓣移植修复。结果术后14例皮瓣全部成活，1例皮瓣远端坏死约4cm×1cm，经换药创面痊愈。10例获随访1个月至2年，皮瓣色泽、质地与外形良好，患者行走及下蹲功能很好。结论带蒂腹壁下动脉穿支皮瓣能修复大腿肿瘤切除术后创面，皮瓣范围大、供区隐蔽，效果理想。
Objective To investigate the effect of repairing the thigh wound from tumorectomy with flap pedicled perforating branch of deep inferior epigastric artery. Methods From January 2002 to January 2009, 15 cases of the thigh wounds from tumorectomy with flaps pedicled perforating branch of deep inferior epigastric artery The area of flaps ranged from 13 cm × 10 cm -40 cm × 10 cm with blood pedicles from 11 - 16 cm. The management of the donor sites is based on the loosening level of defect skin to choose whether suture directly or cover with skin grafting. Results Fourteen flaps survived primarily except for one with 4 cm× 1 cm long necrosis at the distal which was healed by changing dressings. Ten patients were followed up for 1 -24 months, they were satisfied with the color, texture and appearance of the flaps. The functions of walk and squat were satisfactory. Conclusion The flaps pedicled perforating branch of deep inferior epigastric artery can repair large area of the thigh wound from tumorectomy with concealed donor.
