摘要
目的评价村医高血压指南知识、态度和行为(KAB)的培训干预效果,探索适合村医的有效教育干预办法。方法从1117名村医中随机抽取651名进行调查,其中干预组326名,按2005年修订版《中国高血压防治指南》进行干预,比较干预前及干预半年后指南KAB认知率和干预净效应;另312名未进行干预为对照组。结果干预组干预后村医对高血压指南的基本知识和防治观念的认知明显增加,大部分内容增幅和净效应在20%和80%以上。测血压袖带放置部位和教育患者生活方式及降压,虽有提高但差异无统计学意义(P>0.05)。用药最多的是利血平为主的复方制剂,干预后虽下降但仍占55.35%,血管紧张素转换酶抑制剂增加22.68%。对照组各观察指标增幅不明显(P>0.05)。结论培训干预能使村医高血压指南知识观念有明显提高,行为得到改善,但行为的改变相对滞后,说明此类问题是健康教育的难点。
Objective To evaluate the intervention effect of the training project in village doctors＇hypertension guidelines,attitude and behavior for suiting the situation of Chinese village doctors. Methods We got the effect result of intervention through comparing before and after half a year in village doctors＂ hypertension guidelines, attitude and behavior. Meanwhile, we set up a control group for analyzing the net effect of the intervention. Results Understanding of concept had a marked increase to the intervention and treatment and the basic hypertension guidelines knowledge in village doctors, most of the increase and the net effect was over 20% and 80% , respetively.Although improvement in position of blood pressure cuff, patients＂ way of life and reduction of blood press, there was no discrepancy in statistics analysis. In those medicines, the most was ＂Reserpine-based compound preparation＂, declined after the intervention, although still accounted for 55.35%, ＂angiotensin-eonverting enzyme inhibitor＂ increased by 22.68%. The growth of each index in control group was unnoticeable （ P 〉 0.05 ）. Conclusion The village doctors＂ concept of hypertension guidelines knowledge has markedly improved after training intervention, improved behavior, but behavior change has lagged behind, that such issues are difficulties to health education.
