慢性重型肝炎患者入肝血流变化的多普勒超声检测的临床价值 被引量:2

摘要 目的探讨慢性重型肝炎患者门静脉、肝动脉的血流动力学变化。方法根据肝功能受损害的严重程度将患者分成慢性重型肝炎组(28例)及慢性肝炎组(46例),选取20例健康人为对照组。运用多普勒超声检测各组受检者门静脉主干内径(Dpv)、平均血流速度(Vmin),肝动脉峰值流速(PSV)、平均流速(TAV)、阻力指数(RI)及搏动指数(PI),计算门静脉血流量(Qpv)及充血指数(CI),并分组比较。结果慢性重型肝炎患者门静脉管径,充血指数增大,门静脉流速减慢、流量减少,肝动脉峰值流速及搏动指数增加,与对照组和慢性肝炎组比较差异均有统计学意义。结论多普勒超声检测慢性重型肝炎患者门静脉、肝动脉血流动力学改变,对临床治疗、评价病情有重要价值。 Objective To discuss hemodynamic changes of portal vein and hepatic artery in chronic hepatitis gravis. Methods According to the degree of Liver function damage, 74 cases patients were divided into two groups, include chronic hepatitis group(46 )and chronic hepatitis gravis group( 28 ). 20 normal people. Used Doppler ultrasound to detect portal vein diameter ( Dpv), and average blood flow velocity ( Vmin), hepatic artery peak velocity ( PSV), average blood flow velocity ( TAV), resistance index(RI) and pulsatility index( PI), Calculated portal vein blood flow and congestive index(CI). Results Compared to normal and chronic hepatitis patients, Dpv, CI PSV and PI were significantly increase. Qpy and Vmin were significantly decreases in Chronic hepatitis Gravis. Conclusion There was value of cLinical treatment and evaluation of prognosis, to measure hemodynamic changes of portal vein and hepatic artery with doppler ultrasonography in chronic hepatitis gravis.
