摘要
目的探讨腹腔镜阑尾切除时切口设计及选择的若干问题。方法回顾性分析2006年6月～2008年6月在该院明确诊断为阑尾炎而行腹腔镜阑尾切除术的120例患者的手术前情况手术时切口及入路的设计及选择、术后并发症等临床资料。结果全部患者均成功实施了腹腔镜阑尾切除,无中转开腹手术,手术时间30～120min,平均65.2min,住院时间2～9d,平均3.5d,117例患者无术后并发症,2例患者术后出现腹腔脓肿,1例经穿刺引流治愈,1例术后经抗感染治疗,脓肿吸收。1例患者出现术后肠瘘,经持续低压冲洗引流后治愈。结论遵循个体化原则,采取正确的腹部切口设计及入路选择是手术成功的关键。
[ Objective ] To research different incisions for laparoscopie appendectomy. [ Methods ] One hundred and twenty eases patients that were diagnosed appendicitis and were performed laparoseopic appendectomy during June 2006 to June 2008 were retrospectively analyzed. [ Results] All the patients were performed laparoseopic appendectomy. None was converted to open operation. The operative period was 3-120 minutes. The mean duration was 65.2 minutes. The duration in hospital was 2-9 days, and the mean time was 3.5 days. Among the 120 cases, 117 cases recovered without complication; but 2 cases had abdominal abscess after the operation, 1 case had intestinal fistula, and the 3 cases were all cured after corresponding treatments. [Conclusion] Laparoscopic appendectomy will succeed if suitable incisions and individual program are ehosen.
