摘要
目的:了解葡萄球菌临床分离株的红霉素诱导克林霉素耐药情况以及纸片距离对实验结果的影响。方法:收集临床标本中分离的54株(红霉素耐药株、克林霉素中介或敏感)的葡萄球菌,其中金黄色葡萄球菌(Staphylococcus aureus,SA)20株,凝固酶阴性葡萄球菌(coagulase negative Staphylococci,CNS)34株。利用双纸片法检测红霉素诱导克林霉素耐药(D试验),分别测定两种纸片(15mm和26mm)边缘距离,了解纸片距离对D试验结果的影响。结果:红霉素诱导克林霉素耐药的比例为51.90%,其中SA组为70.00%,CNS组为41.20%,P<0.05,分布有明显差异性,纸片距离增大会影响D试验结果的检出。结论:临床微生物实验室应加强对葡萄球菌的红霉素诱导克林霉素型耐药检测,以指导临床合理选用抗菌药物。
Objective: To investigate the prevalence of inducible clindamycin resistance by erythromycin among Staphylococcus isolates and the effect of disk distance on test results. Methods: A total of 54 strains of Staphylococcus resistant to erythromycin and intermediate or susceptible to clindamycin were collected, including 20 strains of Staphylococcus aureus and 34 strains of Coagulase-negative Staphylococcus. The inducible clindamycin resistance by erythromycin (D Test) was tested by double disk diffusion method. Disk distance 15 mm and 26 mm were separately tested to examine the effect of disk distance on the results of D Test. Results: The prevalence of inducible clindamycin resistance was by erythromycin 51.90% in total, 70.00% in S.aureus and 41.20% in Coagulase-negative Staphylococcus, P 〈 0.05. The further disk distance would affect the picking out rate of D Test. Conclusion: The detection of inducible clindamycin resistance by erythromycin in Staphylococcus isolates should be stressed in microbiology laboratory, for guidance of clinical treament of Staphylococcus infection.
